Instructions – items for repair:

1. Click the Download Form button to print out Fault Capture Form and Labels.
2. Print off the form and complete it with as much information as possible regarding the nature of the fault.
3. Telephone +44 (0)1978 853555 to obtain repair number and arrange for collection, indicating number of cartons to be collected.
4. Add allocated repair number to fault capture form and include in each carton with item for repair. Please ensure item is carefully packaged to avoid transit damage.
5. Affix printed address label to each carton.
